- Refer to this guide for program, compliance, or food safety questions.
Temperature Log
- Be sure to record your temps a minimum of every week!
Ohio Food Programs Manual April 2019
- This must be accessible by electronic version or hard copy at all times during your program.
And Justice for All Poster - MUST BE SIZE 11"X17" AND IN COLOR, GREEN: https://www.ascr.usda.gov/sites/default/files/JFAgreen508.pdf
- All programs receiving State and Federal foods must post this in a public area for community members to read.
Written Notice of Applicant and Recipient Rights
- Must be posted at all faith-based organizations except for Senior Box programs.

Effective July 1, 2023 through June 30, 2024

MUST HAVE POSTED:
- Household Income Eligibility Guidelines: English | Spanish
- And Justice for All poster (see above)
- For faith-based organizations, Written Notice of Applicant and Recipient Rights (see above)
Eligibility To Take Food Home Form: English | Spanish
- Keep copies of the current form available in case PantryTrak or the internet isn't working!
Letter of Proxy (must be updated every year in July)
- Use this form if a community member is unable to pick up their own groceries.
