Bridging the gap between food and better health outcomes was the driving need behind this partnership. During a patient’s visit to the hospital, food security is assessed. If the patient is identified as needing food, Cleveland Clinic gives a one-time food box to provide food for today and resources for tomorrow.
Harm Reduction Clinic Food Pantry
Aiming to meet people where they are inspired this program. When someone visits the Harm Reduction Clinic in Lorain, they can now receive a food box that can help fuel their bodies as they take steps to improve their health.
WIC Produce Boxes
Getting more fresh food to mothers and children inspired this collaboration. When families visit their local WIC (Special Supplemental Nutrition Program for Women, Infants, and Children) office, they can take a produce box containing approximately 10 pounds of fruits and vegetables provided by Solidarity Urban Farm in Lorain during produce season or a local grocery store during the off-season.
